Laparoscopic Treatment of a Complication after Endoscopic Lithotripsy for the Common Bile Duct Stones

Abstract
Endoscopic retrograde cholangiopancreatography (ERCP) is the most complex endoscopic procedure for both evaluating and treating bile duct disease. However, it can cause a variety of short-term complications that sometimes requiring surgery to rectify. A 45-year-old male patient was admitted complaining of abdominal pain, fever and jaundice for 3 days. He had mild RUQ (right upper quadrant) tenderness and a high serum bilirubin level (total/direct 15.1/10.3 mg/dl) but there was no fever or leukocytosis. ERCP was performed because multiple stones were found in the gallbladder and common bile duct by abdominal ultrasonography. After removing several common bile duct stones using a basket, mechanical lithotripsy was used due to the presence of a 1.5cm-sized stone. However, the wire broke during the procedure. After failing to remove the broken wire, a decision was made to perform laparoscopic exploration. The wire, remaining common bile duct stone, and gallbladder were removed safely by laparoscopic surgery. The patient was discharged on the 7th postoperative day without complications.
